An ingredient in skin creams sold to protect against sun damage and aging. It has only been tested on pieces of skin in a lab, not on people's faces.

Sold for

Carnosine is marketed in serums and creams as an antioxidant and anti-glycation ingredient that protects skin from sun damage and ageing.

Studies say

Lab models of human skin show carnosine lowers oxidative stress markers after UV exposure, but nobody has shown a visible benefit in a controlled trial on people, so the evidence is C.

Why grade C. For the skin anti-ageing use, the available data are an ex vivo study of human skin explants exposed to simulated sunlight and a 3D fibroblast spheroid model under UVA. No randomized trial on living volunteers is in the citations, so the grade is C. 12 A 2026 randomized trial of oral carnosine in 299 adults measured exercise performance, not skin, and found mostly no effect, so it does not change the skin grade. 3

FDA. Carnosine in skincare is a cosmetic ingredient and sits outside FDA drug approval; no carnosine product is approved by the FDA as a drug for any use. 1

What this means for you

What people take it for
Protecting skin from sun damage and aging.
Can a US doctor prescribe it?
No prescription; it is a cosmetic ingredient.
Properly tested in people?
Not properly: animal or lab studies, or small uncontrolled human reports. A 299-adult trial of carnosine pills looked at exercise, not skin, and mostly found no effect.

What it is

Carnosine is a dipeptide. It is two amino acids, beta-alanine and histidine, joined together. It occurs naturally at high levels in human muscle and brain. Skincare makers use the lab-made form, usually labelled L-carnosine, as an antioxidant additive. 1

In cosmetics it is an ingredient, not a drug. So it does not go through FDA approval. The rule is that the finished product must be safe. It also cannot be sold with drug claims. 1

How it works

Carnosine can mop up reactive oxygen species. It can also bind reactive aldehydes. These molecules attach sugars to proteins and form advanced glycation end-products (AGEs). AGEs are the cross-links blamed for stiff, aged collagen. One lab model used 3D spheroids of human skin fibroblasts exposed to UVA. Carnosine protected the cells there. It also switched on the Nrf2 pathway. This is a cell stress-response program that raises levels of detoxifying enzymes. 21

What human research shows

The citations include no controlled trial of carnosine on skin in living people. The closest human data come from skin biopsies kept alive in culture. An Italian group exposed human skin explants to solar-simulated radiation. They applied L-carnosine to the skin at two concentrations. Carnosine cut reactive oxygen species and AGE formation. It also dulled the changes in a panel of marker genes. Explants cannot report redness, wrinkles or pigment over time. So these are lab results, not clinical ones. 1

The one randomized trial of carnosine in living people tested the oral supplement for exercise performance. It did not test skin. O'Toole and colleagues randomized 299 adults to oral carnosine or placebo for 12 weeks. They measured grip strength, calf raises to exhaustion, a two-minute step test and gait speed. Most comparisons showed no difference. There were two exceptions. Participants under 40 did more calf raises at six weeks. Men over 40 took more steps at 12 weeks. Both were subgroup results. The trial says nothing about carnosine applied to skin. 3

Safety

The cited work links no specific adverse effects to carnosine on the skin. The body also makes it already. Some things are unknown. Do the doses used in skin explants match what reaches the dermis from a store-bought product? Does any effect last with daily use? 12

Questions for your clinician

  • Is there a controlled trial of carnosine on skin in living people?
  • Do the concentrations in skin products match those used in lab studies?
  • Would a proven ingredient fit my skin goal better?

Questions people ask

Is carnosine FDA-approved?

Not as a drug. In skincare it is a cosmetic ingredient, which does not need FDA approval. No carnosine drug product exists in the United States. 1

Does carnosine reduce wrinkles?

No controlled trial in people shows that. The published work uses skin explants and cell models. These measure markers of oxidative stress and glycation, not wrinkles. 12

What does anti-glycation mean?

Glycation is the reaction of sugars with proteins such as collagen. It makes cross-links called AGEs, which stiffen tissue. Carnosine can bind the reactive steps in that process. In a skin-explant model it cut AGE formation after UV exposure. 1

Updates

  • 2026-10-27 added O'Toole 2026 RCT of oral carnosine for endurance (n=299, mostly null; PMID 42308284) to human findings and key studies; grade stays C for the skin use.
