It is a daily pill that lowers LDL (bad) cholesterol. Large trials, including one of 2,909 adults, showed cuts of 56 to 59%, but it is not yet shown to prevent heart attacks.

Enlicitide is talked about as the pill that does what PCSK9 antibody injections do, cutting LDL cholesterol by more than half.
Studies say
Multiple large placebo-controlled trials show LDL reductions of 56 to 59%, with no event-reduction data yet, which is an A for LDL lowering.
Why grade A. Approval rests on the CORALreef phase 3 program of randomized, double-blind, placebo-controlled trials. CORALreef Lipids (n=2,909) showed a 56% placebo-adjusted LDL reduction at 24 weeks, and CORALreef HeFH showed 59% in familial hypercholesterolemia. The endpoint is a surrogate, LDL cholesterol, but it is a validated one for this drug class, and the outcomes trial is ongoing. 13
FDA. On July 15, 2026 the FDA approved enlicitide as Lipfendra (Merck), the first oral PCSK9 inhibitor, as an adjunct to diet and exercise to reduce LDL cholesterol in adults with hypercholesterolemia, including heterozygous familial hypercholesterolemia. Approval was based on LDL lowering; the cardiovascular outcomes trial is still running.

What it is
Enlicitide decanoate is a macrocyclic peptide, a ring-shaped chain of amino acids designed to survive the gut and bind PCSK9, a liver protein that regulates how many LDL receptors sit on liver cells. Oral peptides are hard to make because digestive enzymes destroy them and they are poorly absorbed; Merck's chemists solved this with a rigid cyclic structure and an absorption enhancer, sodium caprate, in the tablet. 12
Until 2026, PCSK9 inhibition required an injection: the antibodies evolocumab and alirocumab every two to four weeks, or the RNA drug inclisiran twice a year. Lipfendra is the first tablet in the class. It was approved on July 16, 2026 for adults with high LDL cholesterol who need more lowering than statins and diet provide. 23
How it works
PCSK9 binds to LDL receptors on liver cells and marks them for destruction, so the liver recycles fewer receptors and clears less LDL from the blood. Enlicitide binds PCSK9 and blocks that interaction, leaving more receptors on the cell surface to pull LDL out of circulation. The effect is additive to statins, which work by increasing LDL receptor production, which is why the trials tested enlicitide on top of statin therapy. 13
What human research shows
CORALreef Lipids randomized 2,909 adults with established cardiovascular disease or high risk of it, 97% on statins, to enlicitide or placebo for 52 weeks. At week 24, LDL cholesterol was 55.8% lower than placebo in the primary analysis (about 60% from baseline in a post hoc analysis that removed biologically implausible baseline LDL values), and non-HDL cholesterol, apolipoprotein B and lipoprotein(a) also fell significantly. The reductions held through week 52. Adverse event rates, including serious events, did not differ meaningfully from placebo. 1
CORALreef HeFH, in adults with heterozygous familial hypercholesterolemia, found a 59% placebo-adjusted LDL reduction at week 24. CORALreef AddOn randomized 301 statin-treated adults to enlicitide, ezetimibe, bempedoic acid or the two combined for eight weeks; LDL fell 64.6% with enlicitide against 27.8% with ezetimibe, 6.3% with bempedoic acid and 36.5% with the combination. 34
What is not yet known is whether these LDL reductions translate into fewer heart attacks and strokes. For the antibody PCSK9 inhibitors they did, and the FDA accepted LDL lowering as the basis for approval, but enlicitide's own outcomes trial, CORALreef Outcomes, has not reported. 3
| Study | Model | Finding |
|---|---|---|
| CORALreef Lipids | Humans, n=2,909, randomized double-blind placebo-controlled, 52 weeks | LDL cholesterol fell 55.8% relative to placebo at week 24 with adverse events similar to placebo. 1 |
| CORALreef HeFH (Merck release) | Humans, randomized placebo-controlled, heterozygous familial hypercholesterolemia | LDL cholesterol fell 59% relative to placebo at week 24. 3 |
| Lipfendra prescribing information | FDA label, 2026 | Approved as an oral tablet to reduce LDL cholesterol in adults with hypercholesterolemia including heterozygous familial hypercholesterolemia. 2 |
Safety
In CORALreef Lipids the proportion of participants with adverse events and serious adverse events was similar on enlicitide and placebo over 52 weeks. Diarrhea and dizziness were noted as common events in the familial hypercholesterolemia trial. The label requires the tablet to be taken on an empty stomach because food reduces absorption. Long-term safety beyond one year and safety in pregnancy have not been established. 12
Questions for your clinician
- Is there evidence yet that it prevents heart attacks, or only lowers LDL?
- How does it compare with a statin or injectable PCSK9 drug for me?
- What does taking it on an empty stomach involve day to day?
Questions people ask
Is enlicitide FDA-approved?
Yes. Lipfendra was approved on July 16, 2026 for adults with hypercholesterolemia, including heterozygous familial hypercholesterolemia, as an add-on to diet and exercise. 23
How much does enlicitide lower LDL cholesterol?
About 56% more than placebo at 24 weeks in the main trial, on top of statin therapy, and 59% in people with familial hypercholesterolemia. That is in the same range as the injectable PCSK9 antibodies. 13
Does enlicitide prevent heart attacks?
Not yet shown. The approval is based on LDL lowering. The cardiovascular outcomes trial, CORALreef Outcomes, is ongoing. The injectable PCSK9 inhibitors did reduce events in their outcomes trials, which is why LDL lowering was accepted as the basis for approval. 3
Is enlicitide a peptide?
Yes, a cyclic peptide chemically modified so it can be absorbed from the gut. It is one of the first orally active peptide drugs targeting a protein-protein interaction instead of a receptor. 1
