A drug given during dialysis to lower a gland hormone that rises in kidney disease. Three controlled trials show it lowers that hormone, but none tested whether it prevents fractures or deaths.

Sold for

Etelcalcetide is an intravenous calcimimetic that lets dialysis units control parathyroid hormone without relying on patients to take pills.

Studies say

Three randomized trials show it lowers parathyroid hormone more than placebo and more than oral cinacalcet, an A for the approved biochemical indication.

Why grade A. Approval rests on two placebo-controlled phase 3 trials (1,023 patients combined) and a head-to-head trial against cinacalcet in 683 patients, all randomized and double-blind, all showing larger reductions in parathyroid hormone. The endpoint is a laboratory marker; no trial has shown that lowering PTH with etelcalcetide reduces fractures or deaths. 12

FDA. Approved. Etelcalcetide was approved by the FDA in February 2017 as Parsabiv (Amgen) for secondary hyperparathyroidism in adults with chronic kidney disease on hemodialysis. It is given intravenously through the dialysis circuit at the end of a session. 2

What it is

Etelcalcetide is a lab-made peptide with eight amino acids. Seven of them are D-isomers, which are mirror images of the natural forms. This protects the peptide from breakdown by enzymes. A disulfide bond links it to a single L-cysteine. It is the second calcimimetic drug after cinacalcet. It is the first that can be given intravenously. 12

In advanced kidney disease, the parathyroid glands enlarge and make too much parathyroid hormone (PTH). Low calcium, high phosphate and low vitamin D trigger this. The result is secondary hyperparathyroidism. It weakens bone and adds to calcium buildup in blood vessels. Parsabiv was approved in 2017 for hemodialysis patients with this condition. 2

How it works

The parathyroid gland senses blood calcium through the calcium-sensing receptor. Etelcalcetide binds that receptor at a different site from calcium itself. It makes the receptor more sensitive. The gland then acts as if calcium were higher than it is, and it makes less PTH. Staff give the drug at the end of dialysis, and dialysis clears most of it. So its blood levels stay steady between sessions. Lowering PTH this way also lowers blood calcium. That is the cause of its main side effect. 12

What human research shows

The label pools two identical placebo-controlled trials. In them, 1,023 hemodialysis patients with secondary hyperparathyroidism were randomized to etelcalcetide or placebo for 26 weeks. In each trial, 77 to 79% of etelcalcetide patients had a PTH drop of more than 30%. On placebo, 11% did. Corrected calcium also fell. Phosphate fell a little (significantly in one of the two trials). 2

The head-to-head trial randomized 683 patients for 26 weeks. One group got intravenous etelcalcetide plus oral placebo. The other got oral cinacalcet plus intravenous placebo. A PTH drop of more than 30% occurred in 68.2% of etelcalcetide patients and 57.7% of cinacalcet patients. That met the noninferiority goal and then the superiority goal. A drop of more than 50% occurred in 52.4% versus 40.2%. Both drugs caused nausea and vomiting at similar rates. 1

None of these trials measured whether patients do better. Cinacalcet's own outcomes trial (EVOLVE, not cited here) failed to show fewer deaths or cardiovascular events. No one has run an outcomes trial of etelcalcetide. 12

Safety

Low blood calcium is the typical adverse effect. In the head-to-head trial, lab tests showed hypocalcemia in most patients in both arms. About 5% on etelcalcetide had symptoms: muscle cramps, tingling, and in rare cases seizures. Low calcium can prolong the QT interval and worsen heart failure. So the label requires calcium monitoring. It advises caution in patients with seizure disorders or heart failure. Nausea, vomiting and diarrhea were the other common events. Dialysis clears the drug, so it is not suitable for patients who are not on hemodialysis. 12

Questions for your clinician

  • Does lowering PTH with etelcalcetide reduce my fracture or death risk?
  • How is my calcium checked after each dialysis session?
  • How does it compare with taking cinacalcet pills?

Questions people ask

Is etelcalcetide FDA-approved?

Yes. Parsabiv was approved in 2017. It treats secondary hyperparathyroidism in adults with chronic kidney disease on hemodialysis. 2

How does etelcalcetide differ from cinacalcet?

Both make the calcium-sensing receptor more sensitive. Patients take cinacalcet by mouth. It is a small molecule. Etelcalcetide is a peptide. Dialysis staff give it by vein. In the head-to-head trial, etelcalcetide lowered PTH in more patients. Rates of nausea were similar. 1

Does lowering PTH with etelcalcetide prevent fractures or heart problems?

No one has shown that. The trials measured PTH and other blood markers, not health outcomes. Cinacalcet, the oral calcimimetic, failed to show a mortality benefit in its outcomes trial. Etelcalcetide has not had one. 12

What is the main side effect?

Low blood calcium. It can cause no symptoms, or it can cause cramps and tingling. In rare cases it causes seizures or heart rhythm changes. Staff check calcium during treatment. 2
