It is prescribed by anti-ageing clinics to raise growth hormone for fat loss, muscle and sleep. It does raise the hormone, but no trial has tested it for those goals in healthy adults.
Sold for
Sermorelin is sold by anti-aging and men's health clinics as a gentler way to restore youthful growth hormone levels, promising fat loss, muscle, deeper sleep and recovery.
Studies say
Sermorelin does raise growth hormone and IGF-1, and small physiology studies show GHRH increases slow-wave sleep, but the fat loss and muscle data come from trials of tesamorelin in HIV patients, and no trial has tested sermorelin for anti-aging in healthy adults.
Why grade C. For the use sermorelin is sold for today, raising growth hormone in healthy middle-aged adults to lose fat, build muscle and sleep better, there is no randomized trial of sermorelin. The controlled human data are physiology experiments in 7 to 13 people using GHRH, a retrospective series of 14 men on a combination regimen, and randomized trials of a different GHRH analog, tesamorelin, in HIV-associated belly fat (806 patients pooled) and in 60 obese adults. Class evidence from another drug in another population does not lift sermorelin itself above C. 1234567812
FDA. Sermorelin was FDA-approved as Geref for diagnosing growth hormone deficiency and for treating growth failure in children with idiopathic growth hormone deficiency. EMD Serono discontinued Geref in 2008, and in 2013 the FDA determined it was not withdrawn for reasons of safety or effectiveness, which allows the agency to approve generic versions, though none has been approved. There is no FDA-approved sermorelin product on the market; what clinics prescribe is compounded, and the anti-aging and body composition uses it is prescribed for were never approved.

What it is
Sermorelin is a synthetic copy of the first 29 amino acids of growth hormone-releasing hormone (GHRH), the hypothalamic hormone that tells the pituitary to release growth hormone. Those 29 amino acids are the fully active portion of the 44-amino-acid hormone. It was approved in the 1990s as Geref, as an injection to test pituitary function and to treat children whose growth hormone secretion was inadequate. The manufacturer stopped making Geref in 2008, and no company has brought a sermorelin product back to market. 10
Because the FDA determined Geref was not pulled for safety or effectiveness reasons, compounding pharmacies may prepare sermorelin, and it has become a staple of telehealth and anti-aging clinics, often combined with growth hormone secretagogues such as ipamorelin or GHRP-2. A 2026 review of performance peptides places sermorelin in the tier of compounds with regulatory-grade data for a different indication and online protocols that have never been tested. 110
How it works
GHRH binds receptors on somatotroph cells in the pituitary and triggers pulses of growth hormone release. Growth hormone then acts on the liver and other tissues to produce IGF-1, which mediates most of its effects on growth and metabolism. Because sermorelin works through the pituitary, the body's feedback loops remain in place: somatostatin, the hormone that shuts off growth hormone release, still acts when levels are high, which is the basis of the claim that sermorelin cannot drive growth hormone as high as injected growth hormone can. Growth hormone secretion declines steadily from early adulthood, a process sometimes called the somatopause, and part of that decline reflects reduced hypothalamic GHRH output, so GHRH replacement is a logical idea. In a 1989 experiment, repeated GHRH dosing in healthy men aged 65 to 78 restored the blunted growth hormone response to a GHRH test. 151314
What human research shows
Sermorelin itself has almost no modern trial data. A 2017 retrospective chart review of men on testosterone therapy who were prescribed sermorelin together with GHRP-2 and GHRP-6 found that in the 14 men who met compliance criteria, IGF-1 rose from a mean of about 160 to about 240 ng/mL over roughly four months. There was no control group, the regimen combined three drugs, and no body composition outcome was measured. 6
The body composition claims come from tesamorelin, a stabilized version of full-length GHRH, in people with HIV and antiretroviral-related belly fat. In a 412-patient randomized trial, tesamorelin cut visceral fat by 15 percent over 26 weeks while placebo patients gained 5 percent, and improved triglycerides. A pooled analysis of two phase 3 trials (806 patients) confirmed the visceral fat reduction, showed it was maintained at 52 weeks in those who stayed on drug, and found no change in subcutaneous fat. A secondary analysis found small increases in trunk muscle density and area. In 60 obese adults without HIV who had low growth hormone secretion, 12 months of tesamorelin reduced visceral fat and carotid artery wall thickness without worsening glucose. A 2026 meta-analysis of five tesamorelin trials in HIV confirmed the visceral fat, liver fat and lean mass effects. Whether compounded sermorelin at clinic doses does any of this has not been tested. 2341112
The sleep claims rest on small physiology studies with GHRH given intravenously overnight. In 7 young men, four pulses of GHRH increased slow-wave sleep from 14 to 20 percent of the night and blunted overnight cortisol. In another study, a GHRH bolus given during the third REM period produced a large increase in slow-wave sleep. In 13 healthy adults in their late 60s, GHRH reduced night-time awakenings but had much less effect on slow-wave sleep than in the young, and did not change cortisol. These are single-night laboratory experiments, not trials of nightly injections. 789

Safety
Sermorelin has a long record of single-dose diagnostic use. The concern with adult anti-aging use is the hormone it raises: growth hormone and IGF-1 excess are tied to insulin resistance, fluid retention, joint pain and carpal tunnel syndrome, and IGF-1 promotes cell growth, which is why a theoretical cancer risk is raised whenever the axis is stimulated long term. The 2026 review of performance peptides lists endocrine disturbances, dysglycemia, fluid retention, musculoskeletal pain and injection site reactions as the adverse effects clinicians encounter in users of this class, and notes the added uncertainty of compounded or unregulated products of unknown composition. No long-term safety study of sermorelin in healthy adults exists. 112
Questions for your clinician
- Is there any controlled trial of sermorelin for the goal I have in mind?
- Could my symptoms come from a treatable cause other than low growth hormone?
- What do raised IGF-1 levels mean for my blood sugar and cancer screening?
Questions people ask
How is sermorelin different from human growth hormone?
Growth hormone injections supply the hormone directly and can push levels as high as the dose dictates. Sermorelin asks the pituitary to release its own growth hormone, so the body's feedback through somatostatin still limits the peak. That is the argument for it being gentler; it is also why it cannot work in someone whose pituitary cannot respond. 15
What does the GHRH research show about visceral fat?
Randomized trials of tesamorelin, a related GHRH analog, in people with HIV showed about a 15 percent reduction in visceral fat over 26 weeks, maintained to 52 weeks, with no loss of subcutaneous fat. A 60-person trial in obese adults without HIV showed a similar effect. No trial has tested sermorelin for fat loss in anyone. 234
Can sermorelin change how you sleep?
GHRH given intravenously overnight increased slow-wave sleep in small studies of young men and reduced awakenings in older adults, with a smaller effect in the elderly. Those were one-night laboratory experiments; whether nightly sermorelin injections improve sleep in practice has not been studied. 789
What is sermorelin's approval history with the FDA?
It was, as Geref, for diagnosing and treating growth hormone deficiency in children. Geref was discontinued in 2008 for business reasons, the FDA ruled in 2013 that it was not withdrawn for safety or effectiveness, and no approved product exists now. Clinics prescribe compounded sermorelin for uses that were never approved. 10
What is the somatopause?
The gradual fall in growth hormone secretion that starts in early adulthood. Research in older men suggests part of it comes from the hypothalamus releasing less GHRH, and repeated GHRH dosing restored the pituitary's response in one small 1989 study. Whether reversing the decline helps or harms healthy older adults is unresolved. 51314
