A self-injected shot for myasthenia gravis, a muscle weakness disease. A trial of 174 patients showed it improved daily symptoms, with a risk of serious infection.
Sold for
Zilucoplan is the first self-injected complement inhibitor for myasthenia gravis, a self-injected shot that replaces hospital infusions.
Studies say
A 174-patient randomized trial showed clinically meaningful improvement in daily-living symptoms over placebo at 12 weeks, with the class risk of meningococcal infection, which supports an A.
Why grade A. Approval rests on RAISE, a 174-patient randomized placebo-controlled phase 3 trial that met its primary endpoint on the patient-reported MG-ADL scale at 12 weeks, plus a phase 2 trial. The approved use matches the trial population, so A; durability beyond 12 weeks comes from open-label extension data. 12
FDA. Zilucoplan is approved by the FDA as Zilbrysq (October 2023) for generalized myasthenia gravis in adults who are anti-acetylcholine receptor antibody positive. It carries a boxed warning for serious meningococcal infections, requires meningococcal vaccination, and is available only through a restricted REMS program. 2

What it is
Zilucoplan is a macrocyclic peptide with 15 amino acids. That means its chain is closed into a ring. It has a fatty acid tail that binds albumin. This helps it last long enough to be injected under the skin. Ra Pharmaceuticals found it with a peptide-screening technology. UCB developed it. 12
Generalized myasthenia gravis is an autoimmune disease in which antibodies attack the junction between nerve and muscle. Most often the target is the acetylcholine receptor. This causes weakness that comes and goes. It affects the eyes, face, throat, limbs and breathing muscles. Before complement inhibitors, doctors used pyridostigmine, steroids and immunosuppressants. They also used intravenous immunoglobulin. For crises they used plasma exchange. 1
How it works
Antibodies stuck to the acetylcholine receptor recruit the complement system. This is a cascade of blood proteins. It punches holes in the muscle membrane at the nerve junction and destroys receptors. Zilucoplan binds complement protein C5. It stops it from being cleaved into C5a and C5b, which blocks formation of the membrane-attack complex. The antibody drugs eculizumab and ravulizumab block the same step. They are given by infusion. Zilucoplan does it with a small peptide that patients can inject themselves. C5 is also needed to kill Neisseria bacteria. So blocking it raises the risk of meningococcal disease. 12
What human research shows
Lancet Neurology published RAISE in 2023. It randomized 174 adults at 75 sites in Europe, Japan and North America. All had generalized myasthenia gravis and antibodies to that receptor. They got zilucoplan or placebo for 12 weeks. The Myasthenia Gravis Activities of Daily Living score is an 8-item scale. Patients report it. It improved by 4.39 points on zilucoplan versus 2.30 on placebo, a difference of 2.09 points. The Quantitative Myasthenia Gravis scale is scored by clinicians. It improved by 6.19 versus 3.25 points. Gains showed by the first week. 1
People from RAISE could join open-label extension studies. The label reports safety data from 213 patients in those studies. Seven of them (3.3 percent) had pancreatic events. 2
| Study | Model | Finding |
|---|---|---|
| RAISE (Howard 2023, Lancet Neurology) | Humans, n=174 with AChR antibody-positive generalized myasthenia gravis, randomized placebo-controlled phase 3, 12 weeks | MG-ADL improved by 4.39 points versus 2.30 on placebo; QMG improved by 6.19 versus 3.25; injection-site bruising was the main adverse event. 1 |

Safety
In RAISE, the most common adverse event was bruising at the injection site. It affected 16 percent on zilucoplan versus 9 percent on placebo. Headache and diarrhea came next. Serious adverse events and serious infections were similar in the two groups. One patient died in each group (COVID-19 on zilucoplan, cerebral haemorrhage on placebo). Neither death was judged related to the drug. No meningococcal infections occurred in the trial. 1
The boxed warning reflects the mechanism. Blocking complement C5 leaves patients open to meningococcal infection. It can be life-threatening. Patients must finish or update meningococcal vaccines at least two weeks before the first dose. If not, they take antibiotics until they have. The drug is dispensed only through the Zilbrysq REMS. Pancreatitis and raised pancreatic enzymes were seen in the development program. They are listed warnings. 2
Questions for your clinician
- Which vaccines are needed before starting zilucoplan?
- How does it compare with infusion complement blockers?
- How long do the benefits last beyond 12 weeks?
Questions people ask
Is zilucoplan FDA-approved?
Yes. Zilbrysq was approved in October 2023 for adults with generalized myasthenia gravis who test positive for acetylcholine receptor antibodies. It is the first complement inhibitor for the disease that patients inject themselves. 2
How does it compare with eculizumab?
Both block complement C5. Eculizumab and ravulizumab are antibodies given by infusion into a vein. Zilucoplan is a small peptide injected under the skin. No head-to-head trial has been published. So whether one works better is not established. 12
How much did patients improve in RAISE?
The patient-reported MG-ADL score improved by about 4.4 points on zilucoplan versus 2.3 on placebo at 12 weeks. That is a difference of roughly 2 points on an 8-item scale. A 2-point change is the usual threshold for clinical meaning. Improvement began within the first week. 1
Why does it require vaccination?
Blocking complement C5 disables the body's main defense against Neisseria meningitidis. This bacterium causes meningococcal meningitis and sepsis. The boxed warning requires vaccination before starting. The drug is only available through a risk-management program. 2
